Thanks I can see any errors - it have displayed all the fields i added and they are working on the form.
SO the log errors give us no clue?

I guess I will have to start removing the code until it doesnt break

phillipines-form?sel=53&oid=3&soid=2&stid=2:671 Uncaught ReferenceError: f is not defined

do you have any element with the name 'f'

and

ff_url_var_init is not defined

do you have active init scrip in the element 'url_var'?

I writing this according to an error message that you have shown in the previous comment

OK - I have found the errors - qll fixed now.
The only thing that is not working is the page before submit - setting the company_id by looking uo the MAX(id). The query is correct but is not setting the company_id field - maybe it is in the wrong place?

Yes 0 I fund those two errors - nthing in consolelog now but error on submit. If you are busy dont worry I wil keep looking and let you know of progress

Good news - I have got it working - the snippet was an issue we started with editing

document.myresubmit.submit();

and the issue was the document.myresubmit.submit()
I had to change it to :
document.getElementById("myresubmit").submit();

So for your info regarding Joomcrm - it has a report maker that create html and the necessary fields and that can be used in the end submit piece with that snippet anc chaning the values to
value="'.htmlentities( ff_getSubmit('email') ).'"
