This is cache problem, please check did you set cache control ok:
extensions.joomla.org/extension/cachecontrol/
Please check Full description, for this joomla cache can be set to on, but if you don't use cache plugin, please set joomla cache to off, otherwise it can cause problem when form is sent (no entry in database)
