I don't recall seeing an option for just exporting/overwriting forms. Next time this happens I'll take a screenshot so you know what I mean.

Anyway, yes please delete the duplicates for me then I'll have a go at step 4 again.

Unfortuantely that's not worked - I can see the duplicate Pieces have been deleted (thank you!) but the pages with the forms on them still aren't working. For example wavescarwash.co.uk/contact-us/feedback.html

What else can we try?

Could you please enable Debug System in System->Global configuration->System, and then come back to me in which line this happend and I will provide you with solution?
