The form isn't anywhere other than embedded in the article. I used this code to embed the form in an article:

this is ok.
This form is in article but it is visible somewhere on your page (I meant that link).

Also, you can disable last page is thank you page, then when someone click on Next button see thank you page, and after that he click on submit button. Then the form will be reload automatic.
