Probably because you seem to have a multi-upload on the form.

The code for this would need to be slightly changed then, also make sure that

"case 'upload':" matches the name (not title) of the upload element:

Thanks, I'm not sure what i'm doing wrong but cannot seem to get this to work for me, with your code above I get an error about cannot get image size, I have tried looking in the forums & have added some code which give me a path in the pdf just cannot get the image to show. My upload field name is "upload" my email field name is "fld_email"
I have attached the template file can you please advise what I am doing wrong.
