It is the same form as displayed on the link.

As noted in form crosstec.org/breezingformsdemo/index.php...pplication-form.html , the look of this form may be completely different depending on the theme you choose.

The theme that is shown there is called "Tycho" theme and it can be found in additiona-themes folder when you purchase one of our subscription plans.

This is how you install some additional themes : crosstec.org/en/support/online-documenta...uickmode-themes.html .

Let me know whether you have managed to set this for your form.

If you sign up for a membership, you'll have access to the "Tycho" theme—as displayed—in the extra-themes folder.
