try like this :

Update:
If you want add time, data, Id and who is submitted form, then try like this:

Thanks Mirko.

Isn't the "username" a logged in user though? The form is a public facing one and there are no logged in users. All I am collecting is their name, phone and email.

If there is a way to place the "fullname" element in the title, that would be great.

Yes, it is a logged-in user.


You can do it on the way that in your export.php file instead of this code

add this code :

please, in this part of code
$db->setQuery("SELECT value FROM fkztj_facileforms_subrecords WHERE Id=(SELECT max(id) FROM fkztj_facileforms_subrecords where name='nameOfElement');");
inser correct SQL Query which will be show values of element which you want.(currentlly SQL query will be display last record stored in database in element 'nameOfElement').

In attach file I send you an export.php file with the correct code only you need to change the SQL query.

It works for the first export, but with a bulks PDF export in the backend results in the same name for all the records.
