This form provides a custom login and the possibility to add more form fields. By that you may require additional information right on login to your website (and even require a captcha for logins). Please note that this form does only work with Joomla! starting from 1.6 (2.5 is recommended).
Installation:
1. Download and unzip the form
2. In BreezingForms go to Configuration => Package Installer
3. Upload the xml from the zip
4. Go to manage forms, select the right package, enter the form and test/edit it
Then create a breezingforms menu item on the form "loginform".
