This is a simple contact form that includes a math captcha. You can install and extend this form as desired, keeping the math captcha. Please leave the 2 fields "question" and "answer" as is. Also make sure you don't change the Before Form and End Submit Pieces code if you intend to use pieces. The captcha is checked with an ajax request and also serverside on submit.
Installation:
1. Download and unzip the form
2. In BreezingForms go to Configuration => Package Installer
3. Upload the xml from the zip
4. Go to manage forms, select the right package, enter the form and test/edit it
