Do not change the name of the form, sections or buttons. These names are needed to trigger the initscript of the form.

The script can be found in "manage scripts" -> select package "custom"-> "Wrapper Script Add Delete Rows" -
The titles can be changed as needed, and you can add as many elements as you need to the sections.
Installation:
1. Download and unzip the form
2. In BreezingForms go to Configuration => Package Installer
3. Upload the xml from the zip
4. Go to manage forms, select the right package, enter the form and test/edit it
